Survey Design 101

I know how difficult it is to do user research, so I usually participate in any surveys (especially short ones) that people ask me to. Today, I ran into a short survey on mobile phones. The survey was nice and short, but unfortunately I hit a brick wall right away:

Bad Survey Design

It’s pretty easy to make an online survey these days (I personally really like Wufoo.com). There is really no excuse for me to even see question 6 on this survey. A “No” on question 5 should have automatically hid question 6 from me. The second best thing, of course, would be to have a “0″ or “Not Applicable” answer on Question 6.
